How is your email setup? Are you using POP connector? If so your ISP may be
able to filter spam. This won't be the case if your using MX records to your
SBS server.

Have a look at GFI's Mail Essentials at www.gfi.com

I've been using IHateSpam Server Edition since June at two accounts quite
successfully. Only negative is that there smallest increment is 25 users for
around $400. However, if others would contact them and ask for a smaller
user license increments, perhaps they would be willing to reduce their price
accordingly.

A commercial spam filter is your best bet. The spam filtering capabilities
in Exchange 2000 are very limited (Exchange 2003 is better though). I'd
recommend doing a Google search on the topic (if you haven't already) as
there have been lots of posts on this subject and you can get broader
perspective. I just wreastled with this a couple of weeks ago when I
started getting inundated by these MS update spoofs. I tried GFI's
MailEssentials 9 but was disappointed with it's performance. I'm currently
running a trial of Vamsoft's Open Filter Relay, enterprise edition, and it
has proven much more effective (and was easier to set up). I've also heard
good things about IHateSpam Server Edition but haven't tried it (yet).
