Do anyone know if a SBS 2003 client with one CAL can have more SQL
instances?

AFAIK there is no limit to the number of SQL Server instances you can
run on the server.
Access is still covered by the SBS CAL.

Henry is correct... there is a SBS Premium-specific clause in the
Supplemental EULA that allows you to do that:

2.2 With respect to Microsoft Windows Small Business Server 2003 Premium
Edition you may install any number of instances of the SQL server portion of
the Server Software on that Server. An "instance" shall mean a running copy
of the SQL Server portion of the Server Software.
</quote>

TTBOMK-> You can't do this (legally) with plain-vanilla SQL. So, one more
reason why SBS is so cool :-)

With vanilla SQL, you need Enterprise to be licensed for multi-instance.
