As far as I can see once you have item-level permissions in place, the only
place you can amend them is at item-level - i.e by changing the permissions
for each item.
You can create a group for your needs which has the permissions you want so
you don't have to add people one at a time to each item, but I can't see how
you can specify the group at a higher-level and expect it to act on
lower-levels (items) as the whole point is that lower-level permissions
over-rule the higher-level permissions.
