Integrated Marketing Campaign Tracking Site Template

Has anyone been able to get the Integrated Marketing Campaign Tracking site
template to work. I have tried it myself and have went to various other SP
site that have samples/demos of the site templates. They all get the same
error when you click on the Campaign Status Forms (Campaign A or B):

"The following location is not accessible, because it is in a different site
collection:
http://spd-tsa2/gchan_imc2/Campaign%20Status%20Forms/Forms/template.xsn"

If you tried to relink it in SP, you still get the error. If you try to
open it with InfoPath (client) you get a template not found error.

Do you have a URL for the download of this template?

I don't seem to have it listed in my list of WSS 3.0 application
templates that I thought was complete.
