How do I request a certificate with multiple CNs using IIS certificate
request wizard?

The CA is a sub-ordinate root CA running Microsoft Certificate Server which
is bundled with Server 2003 Std. Edition

You cannot.

I'm not sure if a certificate with multiple CNs is possible at all. A
wildcard certificate is as close to that as you can get.

You cannot use the IIS wizard to do this, you would have to craft a custom
request:
