I also did this - but now I have a printer which can print the first page
from a different tray and this makes a single document desirable.
However the Broadgun pdf machine will concatanate pdf's - so you could
create a pdf of the cover and one of the body and concatantate them. This
might also help if the printer driver does not have a 'change tray after the
cover' facility. Then print from Adobe Reader.
