Are you selecting "or submit an advaned certificate request"? on the page
/certsrv/certrqus.asp
You then need to select
"Submit a certificate request by using a base-64-encoded CMC or PKCS #10
file, or submit a renewal request by using a base-64-encoded PKCS #7 file"
You then paste in the contents of the certificate request file "certreq.txt"
In the "Certificate Template" section of the page you select "Web Server" -
do you see the others?

you can control what is visible in the list from the "Certification
Authority" MMC in Administrative Tools on the server. Under your domain
there is a node called "Certificate Templates" right click and select
"New" -> "Certificate Template to Issue" and you can choose other
certificate types.
You can also process your certificate request from within this MMC tool but
this requires some additional attributes (that I do not know about) this is
the warning I received using the normal web server certificate from within
the MMC.
