making a form work??

I am trying to create a simple form that routes
information to an ms access database:
http://www.dekalbhealth.net/test/form-experiment.htm i
have also created another one at
http://www.dekalbhealth.net/test/form-experiment.asp


problem: how is it possible to upload files to my server
through this form? shouldnt frontpage be able to make this
work??

am i doing something wrong?? i am a designer not much of a
developer sorry

The database should be in your web in a folder called _fpdb

Microsoft has made this a lot harder than a rational
person might think it should be. Browse this URL to

By contrast, this sort of thing is fairly easy to do if
you can write ASP or ASP.NET code.
