Your host must configure the forms to email transport in the FP Server
Extensions on the host server.

Have they?

Checking the page will not confirm whether or not the host has the FP server
extensions enabled for forms to email. They would have to tell you that.
You would have to ask them to do it.

If they don't know how to do it, then I suggest you have a host that doesn't
know what they're doing, and you should change.

FP 2002 server extensions are the most current extensions.

You can not use spaces in field names (like "Birth Month")
- remove all spaces (and ampersands - &) from field names (or replace w/ under_scores)

PS
I would never fill out that form on a non-secure server
You are asking for critical personal; information that anyone may be able to Hack or intercept
If I were you I'd get a professional to do it w/ server side scripting on a secure server
