I made a web form and on it towards the top there is the
following:

How can I remove that?

Also is it possible to edit the form confirmation screen
to return to a different page then the form again?

That will go away when you publish your web to a server running the
FrontPage extensions.
As for the a custom Confirmation page, right click your form, click Form
Properties, click Options, click Confirmation Page,
Browse to and select any page you want.

Must be http:// published to a server that is running the FrontPage server
extensions. If you are trying to email the results, the host must configure
the forms to email transport in the extensions on the host server.
