I want to hire someone to build a form for me.

I created the pages using FP as the html editor, but I didn't publish
through Frontpage, nor did I use FP to "build" the website.

The web host 1and1.com does offer Frontpage extensions. I don't have them
turned on at the moment but its simple to do.

The form will be similar to:
http://www.iampeth.com/2005_convention/registration_2005.pdf

Will it be possible to integrate this a form like this into my website?

The FP extensions are not needed to create the form. What
you will need is someone to create the server side script to
read the form and do what you want with the information.
This site is running on Apache- if it was using IIS I would
be glad to assist you
