I would like to create a custom form to collect recipes from several forums
I subscribe to. I have tried to use the "Form Wizard" and the "Office
Assistant" with no luck. How can I create a form to collect this
information?

I'm not sure what you mean by "collect recipes from forums."
Are you wanting people to fill out a form, or are you trying to capture content / posts from the
forum??

You need to click File > New and then select Formpage wizard from the right panel.
Follow the steps in the applet (just keep clicking Next and then Finish.
Then right click in the area that contains the Submit and Clear buttons, click Form properties then
Options and then the Saved Fields tab and start entering the fields you want filled out.

Every item you want the user to enter requires a field.

However, since recipes vary so much I would think it would be easier to just have them send an
email.

And the forms pages won't work unless they are published as part of a FP web in http mode to a server hosted site running the FP
Server Extensions w/ Email transport enabled
