Address bar (and IE Tab) icon-like image

This is probably a root-basic question, but, I have been noticing a small
image in place of the IE "page" icon in the address bar and the Tab on some
sites. In fact, this one has a white "M" left justified in a blue
round-cornered square.

I would like to add a small logo that I have in my site.

How?

Thanks for your answer, Dave, but I have the icon already. I just would like
to know how to get it to appear in the address bar and the page tab.

I couldn't find that on the site that you gave.
