I'm assuming you mean the Active Directory domain when you say "internal
domain". By default Exchang picks up the AD domain to generate email
addresses. You can create an Accepted Domain to add your external/registered
domain, and an EmailAddressPolicy to generate email addresses for users
(make it the default email address, this is what email gets sent as).
