Since I upgraded my powerbook with office SP2, the alerts daemon has
been throwing calendar / office notification alerts up at random
intervals, somewhere between 10-30 at a time. All of these
notifications have been previously dismissed - most of the appointments
are a week or more old. Every 5-10 minutes, however, I get 30 more
alerts.

The only configured account is to an exchange server.

Notifications have worked properly up to now.

Anyone else see anything like this?

I had a similar problem. I was finally able to dismiss all of the messages
but for a while the Office Notifications daemon would crash when I tried to
dismiss them. All of the notifications were from my Exchange calendar, not
my local calendar. As much as I would like to believe that SP2 has improved
Exchange compatibility, there is still a lot left to be fixed.
