One of the options when classifying a message as 'not junk' is to create a
mailing list rule because 'messages handled by a rule will never be
classified as junk', right?

So how can a message that is sent, by a rule, to a particular folder on
arrival, end up there classified as junk?

The order of operation is:
1. Mailing List Manager
2. Junk Mail Filter
3. Rules

So, a message handled by the MLM can only be marked as junk if the MLM
option to 'allow other rules to act on this message' is set on.
