Previously, with our manual method of installing a file system filter
driver, I could go into Device Manager, right click on the top tree entry,
select "Show Hidden Devices" and a "Non Plug and Play Devices" entry would
magically appear. Our device driver would be there. From that I can
stop/start and set the start type.

I've just gotten a device install working with DIFxApp, and it doesn't seem
to be available there. Is this the way it is supposed to be? How do I change
the start type to manual (for instance), without going into regedit?

I'm not sure what your previous manual method consisted of. Have you tried
installing the driver using DPInst? That will give us a good baseline of
what to expect. DIFxApp should have the same affect on the Device Manager
list that DPInst does. Both tools should add an item to the Add/Remove
Programs list for the driver package. When using DIFxApp, an item will
also be added to the Add/Remove Programs list for the application.

You should set your 'Start Type' in your INF file.
