You can verify that it's a popup blocker issue by trying to open
http://<server>/loader.aspx. If this page loads OK, then it is a popup
blocker. Some of the toolbar popup blockers do not seem to recognize the MS
CRM server, even when it is placed in the site list of the utilitiy. But,
if there are multiple installed, then you need to configure each of them to
allow CRM.
