Not that familiar with the form area of CRM but I am trying to create a new
tab (within a form) that would have a radio button with text beside or below
it.
Ex: yes no (radio button)
This statement typed beside or below:
lead qualification criteria complete. (Willing to Explore Solution, Agree
upon actions towards technical proof set into the future, and Product need
enunciated).

Is this possible? If so how?

Create a textarea and set the text "lead qualification criteria complete.
(Willing to Explore Solution, Agree upon actions towards technical proof set
into the future, and Product need enunciated)" as the default value.

Then add the textarea to the form.

So are you saying I should some how create an Attribute with this
information? I think I may need a play by play. Help

Hi Elisa,

Yes. Here are the steps:

1. Click on Settings->Customization
2. Double-click on the entity in which the attribute is to be added
3. Click on the 'Attributes' link on the left navigation of the entity
customization form
4. Click 'New' and create a new text area. Provide the text you want as the
default value of the text area.
5. Save and Publish the Customization.

What Type of attribute?
Type: nvarchar
Format: Textarea
Max length: .........

I don't see anything that says Default Value. I need more information.
Please help.

sorry,. it's not possible for textfields (ntext / nvarchar) to set a
default value.

the only way to show text without programming / coding is to use
section headings.

hy elisa,
here ist the solution for you:

create a text field ntext for your question (new_question1). create
your answer field (new_answer1) as bit for your yes/no option (or
checkbox).
put both fields in the form.

Edit field properties:
new_question1: don't show the name in the form, disable field in form,
adapt the number of rows to the size of your text
new_answer1: don't show the name in the form

edit the on load event of the form:

var question1 = "fill in your question here";

crm.Form.all.new_question1.DataValue = question1

save, publish form.

be careful, the script is very sensitive for Caps!!!!

the solution: here it is:
(I have done it today, so I know its working...)

1) create ntext field for your question (new_question1)
create bit field for your answer (new_answer1)

put both fields on your form

2) edit the onload event of the form:

var question1 = "fill in the text of your question here";

crmForm.all.new_question1.DataValue = question1

3) edit the field properties:

question1: don't show lable on form
disable field on form
adapt number of rows to lenght of your question (try 2, you can change
it later if necessary)

answer1: don't show lable on form

4) for more questions and answers do the same for question2,
question3, and so on...
be careful with caps!!!! the script ist case sensitive!!!
