Settings and Customization will disappear if you have Outlook open and
running CRM sometimes - close Outlook, refresh, if that doesn't work, clear
your cookies and try again. Because we are going to need to go into Settings
and check - it all looks like you do not have permission to create any type
of record at all - that definitely will cause the problems you have in
Outlook.
As I recall, it's Settings, Business Unit Settings, Users. Open your user,
and click on Roles in the left pane under Details. If you are administrator,
you should have permission to add roles - I believe the button is called
Manage Roles? From there, find some roles that apply to what you want to
test, and apply them to yourself.
I don't know if there's any propogation time or anything, a full browser
refresh hopefully will show a bunch of new buttons.
