I created a new Word Mail Merge template in CRM 4.0. After saving the new
template record, I used Create Template in Word to create a template and
saved it as .xml on my desktop. I then attached this xml file to the template
record.

However, when I go to use it from Accounts view, I do not see the Addin menu
in Word 2007.

if you are using web client then close the outlook as it must not be
running. It will be either additional tab or it will be in
"mailing".
