Hi Paula,
As I remember, when an activity is created from with in a case, its
regarding field is prefilled with the case and so it will endup with the case
and not with account or contact. Regarding field in the activity decides
where to show the activity.
May be somebody have changed the default behaviour by doing some
customization on case entity. You need to changed the mapping in case
accourding you to your requirments.
