If I open my Address Book while in Word or Outlook and choose CRM Accounts
from "Show Names From" dialog box quite a few Accounts are missing even
though I sync, go online or offline and close the web client. What controls
the population of this address book?

Two things: access to the instance data and an email address on that
instance. Items in the address book must have an email address or Outlook
won't show them (so the AB doesn't download them). Plus, CRM security kicks
in as you would expect.
