I am looking to create a single activity to multiple accounts, or contacts.

The only activity that really allows for a one-to-many relationship is the
email activity because you can CC: many other contacts, users, leads, etc.
Most of the activities let you have a From:entity, To:entity, and a
Regarding:entity. That's about it. There's no other way to establish
one-to-many relationships for activities.

Well... in addition....letter, fax, phone call and appointment activities
can all have multiple recipients/attendees

Could you tell me how one would set a follow-up call to multiple contacts or
leads? The help file in CRM explains the following but doesnt tell you how
to perform the task. Any suggestions...

Using Microsoft CRM, you can:
Associate a single activity with multiple accounts, contacts, leads, or
opportunities.
